Colorado RHTP: Whole Person Health and Alternative Care Models

Requirements Checklist

• Applicants must demonstrate plans for implementing, training, and building infrastructure for whole-person health and alternative care models to improve access and sustainability. LAOs are responsible for application, contract execution, and fiscal management. The project narrative must address five-year goals but detailed work plan and budget are required for Budget Year One (~12 months). Requirements include active collaboration with relevant advisory boards, partner agreements, and participation in information sharing and outreach. Applicants must upload a Certificate of Good Standing, current W-9, compliance with federal audit requirements (if applicable), and an attestation letter for collaborative/third-party applications. All activities and spending must occur within Colorado’s rural/frontier counties.
