Colorado RHTP: Strengthen and Expand the Rural Health Workforce

Requirements Checklist

• Applicants must propose activities to recruit, train, strengthen, or retain rural healthcare workers. Projects should directly address barriers to workforce recruitment, well-being, and retention, such as housing, childcare, and transportation. Proposals must include a five-year program narrative and annual budget, demonstrate alignment with the Rural Health Transformation Program's aims to improve equity, access, and outcomes for rural populations, and adhere to RHTP guidance on eligible activities and expenses.

Required attachments

• Program narrative (5-year project overview)
• Budget and work plan (Year 1)
• Proof of eligibility (Lead Applicant qualification)
• Letter(s) of Attestation for collaborative applications
• Certificate of Good Standing
• W9
• Letters of support (if applicable)
• Documentation addressing workforce barriers (housing, childcare, transportation as relevant)
