Colorado RHTP: Value-Based Care and Rural Workforce Grants

Summary

the Rural Health Workforce. Supports provider transition to value based payment models and workforce recruitment and retention in rural and frontier communities.

Eligibility

Eligible lead applicants include qualified rural health care providers such as CAHs, REHs, rural PPS and sole community hospitals, FQHC or look-alike, RHC, CMHC or CCBHC, OTP, physician or NPP groups, and EMS organizations. Collaborative and third-party applications require required attestations.

Requirements Checklist

• Applicants must plan, train, and build infrastructure for whole-person health and alternative care models; engage in statewide advisory board and outreach activities; coordinate and improve EMS services; expand rural health workforce; and utilize funds for expanded services, technological improvements, strengthened delivery partnerships, and workforce support (e.g., housing, childcare, transportation). Participation in program evaluation and collaboration is expected.
