Colorado RHTP: Mini-Grants to Rural Hospitals

Summary

Flexible RHTP mini-grants for rural hospitals to expand services improve technology strengthen partnerships and support workforce well-being.

Eligibility

Eligible lead applicants include qualified rural health care providers such as CAHs, REHs, rural PPS and sole community hospitals, FQHC or look-alike, RHC, CMHC or CCBHC, OTP, physician or NPP groups, and EMS organizations. Collaborative and third-party applications require required attestations.

Requirements Checklist

• Applicants must demonstrate how mini-grant funds will be used to expand clinical or community services, enhance technology, foster partnerships, or strengthen workforce well-being. Required materials may include organizational eligibility documentation, a budget, a project narrative, and compliance with RHTP program and advisory input.
