Required elements include: accountability for end-to-end staffing pool performance (clinician participation, shift fulfillment, workforce utilization, reporting); inclusion of all operational, technological, and workforce components with documented commitments between partnering entities; detailed line-item budget and narrative using RHTP DLIB guidance; adherence to milestone requirements (lead entity within 30 days, MOU with partners within 60 days, initial clinician recruitment within 60 days, operational workflow within 120 days, innovation strategy deployed within 150 days); real-time data integration and reporting with State data platforms; all funding must be spent and invoiced by July 31, 2027. The application must document provider participation, geographic scope, and workforce need, and will be reviewed for scalability, innovative workforce approaches, and cross-organization collaboration.
