Summary

The program is tied to milestone deliverables and structured to promote regional scalability and cross-system collaboration. All data, reports, and operational products must be accessible to the State in real time for oversight, analytics, and reporting.

Requirements Checklist

• A single, integrated application must cover governance, workforce strategy, operational model, and supporting technology or partnerships. Required elements: lead operating entity established within 30 days; partnership MOUs within 60 days (if applicable); initial clinician recruitment within 60 days; operational workflow in place within 120 days; at least one innovative staffing strategy implemented within 150 days; evidence of provider participation, geographic scope, and workforce need; and detailed line-item budget with narrative per RHTP DLIB guidance. Clinician participation, shift fulfillment, workforce utilization, and performance reporting are required. All pools must demonstrate measurable improvement in staffing coverage, workforce utilization, access to services (including use of telemedicine), operational efficiency, and retention. Applications will be reviewed for scalability, innovation, alignment with RHTP objectives, and cross-system collaboration. All data must be integrated into the State’s Connected Care Grid Data Spine or interim system. Award funds must be spent and invoiced by July 31, 2027.
