Requirements Checklist

• Applicants must submit: (1) proposal narrative, (2) detailed line-item budget worksheet and budget narrative (following RHTP DLIB instructions), (3) required forms (including county-level service and grantee contact information), (4) proof of WV Oasis registration, (5) letters of commitment from key partners (if applicable). Applications must detail new spoke sites, populations served, operational and sustainability plan, and strategies for data integration and compliance with CMS/HHS and State requirements. Funding is milestone-based: 25% initial advance (start-up), 25% on hub/half spokes operational and data reporting, and 50% on full spoke network and WVHIN integration. Monthly, quarterly, and annual financial and programmatic reporting is required. Funds may not replace existing activities/budgets and may not be used as match for any other programs without RHTP approval. All activities must ensure integration with the State's RHTP data spine and meet all technical and administrative guidelines.
