The program must not duplicate activities funded by Medicaid MCOs, Medicare, other grants, or any payer/entity legally responsible. All activities shall support the Governor’s Four Pillars of a Healthy West Virginia, align with RHTP/CMS priorities, and require integration with State platforms and CMS-aligned data/reporting. The primary grantee must demonstrate capacity for milestone-based performance management, sustainability planning, verified engagement and completion of referrals, county-level reporting, and measurable impact within defined timeframes in referral utilization, completion, participant health outcomes, healthcare cost/utilization reduction, and workforce/economic impact. Minimum platform functionality within 60 days of contract execution includes live referral initiation, CBO receipt and disposition, audit-log of referral status, and readiness for real-time State access. All grantees must rigorously document non-duplication controls, data integration readiness (including interface standards and security), and commitment to privacy and data governance. Scalability and sustainability beyond Year 1 must be demonstrated. Applications must clearly justify budget allocations, distinguishing healthcare provider payments (subject to 15% cap) from programmatic/infrastructure costs, and comply with performance-based milestone reimbursement.
Eligible applicants are organizations capable of serving as a single statewide program operator, including entities that own or partner with a closed-loop referral platform and can demonstrate experience in healthcare integration, provider engagement, and performance-based operations. Applicant types include multi-partner collaboratives, coalitions, consortiums (with a single main applicant) with ability to scale statewide, other non-profit organizations, and third-party vendors supporting closed-loop referral platform products with experience/capability to drive implementation, change management, and platform use/performance accountability. FQHCs, CHCs, health systems, and direct-service nutrition/wellness/activity providers are not eligible as direct grantees but may join as key partners. Applicants must demonstrate operational ownership and readiness at time of application, proof of partner engagement, and ability to comply with milestone-based performance and data integration requirements.
