NSVRC requests applications for projects that establish, expand, or deploy sustainable mobile and hybrid care models serving eligible rural residents in its Virginia Rural Health Transformation Program service area. Eligible approaches include but are not limited to mobile care units, telehealth kiosks, clinics in a box, electronic consultations, rural care hubs, hub-and-spoke models, and technology enablers for rural care. Services supported may include primary care, behavioral health and substance use disorder services, dental care, vision care, preventive care, screenings, diagnostics, and specialty care. Projects must address a documented rural healthcare access need within eligible communities and demonstrate potential for long-term sustainability.
Eligible applicants must be legally organized entities in good standing and capable of receiving and administering a federal subaward (including nonprofit organizations, governmental entities, health systems, Federally Qualified Health Centers, Rural Health Clinics, educational institutions, healthcare providers, community-based organizations, mobile health technology organizations, and other qualified entities). Applicants must have an active SAM.gov registration with a current UEI, operate in or directly serve rural areas in the NSVRC Planning District (Clarke, Frederick, Warren, Shenandoah, and Page Counties and the City of Winchester, as defined by the FORHP rurality definition), and demonstrate sufficient organizational, financial, and administrative capacity to manage a federal reimbursement-based award. If proposing direct clinical services, applicants must hold or formally partner with an entity holding all required licenses and credentials.
