Requirements Checklist

• Applications must include a completed Application Cover Sheet, Project Narrative, Implementation Work Plan and Performance Measures, detailed Budget and Budget Narrative, documentation of active SAM.gov registration and UEI, most recent organizational audit or financial statements, clinical licensure/credentials as applicable, letters of commitment from key partners (as applicable), and required certifications/disclosures. Projects must implement an eligible mobile or hybrid care model serving rural residents, comply with all federal rules and reporting, and adhere to all RHTP and NSVRC requirements for monitoring, reporting, and subaward oversight.

Submission Instructions

Submit one complete electronic application in PDF format by email to bdavis@nsvregion.org by 5:00 PM Eastern on September 18, 2026. Organize the application as detailed in Section 4.2 of the RFA.
