Virginia RHTP Mobile & Hybrid Care RFA — LENOWISCO Planning District Commission (Rural Health Transformation SWVA)

Summary

the Southwest Virginia (Planning Districts 1, 2 and 3) SWVA Regional PDC Collaborative service area. Eligible approaches include mobile care units, telehealth kiosks, clinics in a box, electronic consultations, rural care center hubs, and hub-and-spoke technology enablers; services may include primary, behavioral health/SUD, dental, vision, preventive, diagnostic, and specialty care. Approximately $11.8M available; at least five awards expected; no established maximum award per application.

Eligibility

Applicants must be physically located in, or provide direct services to, rural Virginia communities as defined under Virginia's RHTP (FORHP baseline definition). Projects must target residents of: Bland, Buchanan, Carroll, Dickenson, Grayson, Lee, Russell, Smyth, Tazewell, Wise, and Wythe counties; cities of Galax and Norton.

Requirements Checklist

• Required: 1) Complete and submit Narrative Response (10 pages max), including Project Description, Target Population, Implementation Plan, and Sustainability Approach, as set forth in the RFA section IV; 2) Complete Budget Template (Attachment B); 3) Complete Applicant Certification & Assurances (Attachment C); 4) Letters of Commitment from partner organizations if relevant. All templates and forms must be included and completed as specified in the RFA. Proposals must demonstrate the ability to deliver qualified mobile or hybrid care models and must address Community Need, Project Work Plan, Organizational Capacity, Evaluation Plan, and Budget Justification as per review criteria.
