the Rural Health Transformation Program (RHTP) “Connected to Care, Closer to Home” initiative. The program aims to expand access to health care services, strengthen care coordination, and improve health outcomes in rural Virginia by integrating EMS personnel into patient-centered, community-based care delivery models. Initial subawards (up to $400,000 each) will be competitively awarded within three Regional EMS Council service areas to be selected through application ranking (not preselected). Approximately $3.7 million in federal funds are available for the initial performance/budget period, with potential for continuation funding through September 2030, pending continued availability and satisfactory performance. Eligible applicants include licensed EMS agencies, nonprofit organizations, state, local, or Tribal governmental entities, hospitals/health systems, FQHCs, rural health clinics, institutions of higher education, and others with demonstrated capacity to implement CP/MIH services in FORHP-designated rural Virginia communities. Minimum fundable score to be considered for award is 70/100.
Eligible applicants include licensed Emergency Medical Services (EMS) agencies, nonprofit organizations, state, local, or Tribal governmental entities, hospitals and health systems, Federally Qualified Health Centers (FQHCs), Rural Health Clinics (RHCs), institutions of higher education, and other organizations with demonstrated capacity to implement Community Paramedicine/MIH programs. Applicants must have or be actively obtaining a current SAM.gov registration with a UEI, and must operate in or serve one or more FORHP-designated rural Virginia localities within a Regional EMS Council service area (see Appendix A of the RFA). The three initial implementation regions will be identified through statewide application review (not preselected). Minimum fundable score: 70/100.
