Statewide Local Health Challenge (Personal Health Accelerator)

Summary

The program prioritizes chronic disease reduction, evidence-based interventions, data integration via the State Data Spine, and scalable county-level pilots tracked through milestone achievements. Selected grantee(s) must execute a replicable model, administer performance-based incentives, and coordinate with local health departments, employers, schools, and community organizations to engage high-risk, at-risk, and general populations. Multi-region or statewide partnerships are strongly encouraged, with preference for consortia offering food-as-medicine, nutrition support, or related wellness delivery capacity. Disbursement is milestone-based: initial pilot launches in 2–3 counties, expansion to 5–8 counties in Year 1, and a required path to 100% county participation by Year 5. All grantees must comply with State-defined reporting, budget, and data standards to ensure accountability, privacy, and impact.

Eligibility

Eligible applicants include organizations able to design and operate county-level and statewide health challenge programming in West Virginia, including local health departments, healthcare systems, nonprofits, and entities with population-health implementation capacity.

Requirements Checklist

• Submit a complete application via WV OASIS VSS citing GFO-0506-2600000000260-4 (AFA RHT-AFA-05-27-2026-PHA), including all required forms, detailed line-item budget, budget narrative, grantee contact information, and documents as specified in the AFA and any posted Q&A or amendments. Applications must strictly follow data governance, privacy, milestone reporting, and cost allowability requirements set by the State and CMS.
