South Carolina Cancer Screening Improvement Initiative

Summary

SCORH, supported by BlueCross BlueShield of South Carolina Foundation, is seeking up to 15 rural clinics in South Carolina for the Cancer Screening Improvement Initiative. Selected clinics will implement evidence-based policies, improve closed-loop referrals, and enhance outcome tracking to increase cancer screening rates. Funding supports clinics to adopt best practices, participate in training and evaluation, and share outcomes for rural health improvement. Awardees will receive up to $5,000 and participate in a three-year grant program focused on improving cancer screening in rural South Carolina clinics.

Eligibility

Eligible applicants: South Carolina rural clinics; the first cohort is limited to 15 clinics.

Requirements Checklist

• Applicants must be rural clinics located in South Carolina, able to commit to participation in training, evaluation, and outcomes reporting relating to cancer screening. Funding will support policy and procedure improvements, closing the referral loop, outcome tracking, and participation in collaborative learning. Limited to 15 clinics in the first cohort. Award amount up to $5,000 per clinic.
