This opportunity provides up to $29,500,000 in competitive grants under the West Virginia Rural Health Transformation Program (RHTP) – SmartCare Catalyst Provider Productivity Support Fund (PPSF) for FY2026-2027. The PPSF aims to drive statewide modernization for value-based care among rural hospitals, clinics, and stand-alone provider offices in West Virginia. Funding supports operational transformation, technology investment, service restructuring, and development of shared service collaboratives.
