Smart Care Catalyst – Implementation Support to Strengthen Value-Based Care (VBC) Participation (RHT-AFA-07-27-2026-SCC1)

Summary

The State of West Virginia is releasing up to $1,500,000 under the Rural Health Transformation Program (RHTP) for Smart Care Catalyst implementation support to help rural providers in southern counties build readiness for substance use disorder (SUD)-focused value-based care (VBC) models. This opportunity will fund provider onboarding, technical assistance, workflow redesign, and data integration — targeting practical enablement of providers and payers for VBC participation. Milestone-based subawards will prioritize measurable outcomes, rapid implementation, and regional engagement, including stakeholder assessment, defining provider training needs, and reducing barriers to SUD-focused VBC participation. All implementation support must supplement, not supplant, existing activities and comply with federal and state reporting and data privacy regulations. Activities must be focused in rural and underserved southern West Virginia counties with a strong requirement for integration of data into the State’s RHTP data architecture. Funds will be awarded to one or more qualified grantees able to demonstrate experience in provider enablement, VBC implementation, technical assistance, and healthcare analytics. Continuation of funding beyond the one-year term is contingent on State and CMS decisions, performance, and compliance.

Eligibility

Organizations with demonstrated capability in provider enablement/VBC implementation, care delivery transformation or technical assistance, population health operations or program administration, or healthcare analytics and performance measurement. Must be able to operate in southern counties of West Virginia.
