Requirements Checklist

• Applications must be submitted electronically via the WVOASIS Vendor Self-Serve (VSS) portal. Required attachments include: Application Narrative (addressing required sections), Detailed Line-Item Budget, Budget Narrative/Justification, Target Funding Budgets in Excel, Grantee Contact List, County-Level Service Form, and Indirect Rate Agreement or statement. Applicants must certify compliance with cost caps, non-supplanting, and reporting; register for a UEI (Unique Entity Identifier) and with the WV Secretary of State. Respond to evaluation criteria in the narrative. Documentation verifying that funded activities are non-duplicative and address documented gaps must be maintained.
