This opportunity solicits applications to design, implement, and deliver components of Program of All-Inclusive Care for the Elderly (PACE)-aligned care models for older adults with complex medical and functional needs, particularly those meeting a nursing home level of care, in West Virginia. The primary focus is on the rapid establishment and expansion of adult medical daycare (Adult Day Health Care) services, especially in counties with limited or no such offerings, to enable community-based care, reduce avoidable healthcare utilization, and improve access and outcomes. Applicants must demonstrate the capability to implement scalable models aligned with PACE principles, with measurable impact on care coordination, participant outcomes, and foundational operational readiness. Funded programs must avoid duplication of existing reimbursed services, submit detailed budgets and narratives, achieve strict performance milestones, and fully integrate with the State's data spine and reporting requirements. Collaboration with the Bureau for Medical Services (State Medicaid) and other RHTP initiatives is strongly encouraged to ensure program alignment and maximize impact. Year 1 funding is $1.25 million disbursed in milestone-based payments contingent upon achievement of readiness, implementation, and scalability targets. All activities must be completed and all funds expended by July 31, 2027.
