SCC PACE

Summary

The State of West Virginia, through the Rural Health Transformation Program, solicits applications to design, implement, and deliver PACE-aligned care model components for older adults with complex medical and functional needs who meet nursing home level of care. Applicants must prioritize expansion of adult medical daycare services in West Virginia counties where such services are limited or unavailable. Funded programs must demonstrate measurable improvements in care access, outcomes, and operational readiness within defined timeframes, rapid establishment and expansion of community-based care capacity, and a credible pathway to full PACE program readiness by Year 5. Awardees are required to align with State and CMS performance reporting, coordinate with Medicaid and community partners, and avoid duplication of existing reimbursed services. Total Year 1 funding of $1.25 million will be disbursed via milestone-based payments contingent on achievement of performance targets. Applications must be submitted through the WVOASIS Vendor Self Service portal by September 2, 2026, 11:59 PM ET. Cost-sharing may not be required unless prior written State approval is obtained.
