Rural Technology Transformation Fund: Allocations to Improve Health Services in Rural Wisconsin

Eligibility

Eligible applicants for Round 1 are healthcare organizations with one or more Rural Health Clinics or one or more Federally Qualified Health Centers (FQHC) serving rural or semi-rural Wisconsin communities, as listed in Addendum Exhibit 4 of the opportunity. Only organizations named in Exhibit 4 are eligible for this round. Applicants must have a valid Unique Entity Identifier (UEI) and active SAM.gov registration at the time of award.

Requirements Checklist

• Applicants must submit an online application by 11:59 p.m. Central on August 1, 2026 that includes responses to narrative questions on community needs/impact, program design, sustainability, and a detailed budget in the provided template. Projects must propose technology investments that directly benefit rural or semi-rural Wisconsin residents, and funds may not be used to supplant existing funding, support routine operations, or for unallowable expenses as defined in federal guidelines. Administrative costs are capped at 8% of the award. Minor alterations/renovations are eligible up to 20% of award with prior approval. All applicants must meet federal compliance and reporting requirements outlined in 2 CFR Part 200 and program addenda.
