the Rural Health Transformation Program (RHTP) workforce initiative. Successful applicants will design, deliver, and assess training programs for CHWs and CHW supervisors statewide, including two required components: CHW Core Competency training (minimum 40 hours, competency-based) and CHW Supervisor/Leader training (minimum 20 hours, focused on supervision and leadership in CHW programs). Up to $600,000 per year is available (pending annual CMS approval) across all awards; individual awards anticipated between $150,000 and $300,000 annually. The project period is October 1, 2026 – July 31, 2029. This RFA is for provider organizations delivering CHW and CHW Supervisor training and is distinct from related grants for CHW-employing sites.
Eligible applicants include educational institutions, nonprofit organizations, healthcare systems, and tribal entities based in Wisconsin with demonstrated experience in developing and delivering competency-based CHW and CHW supervisor training. For-profit organizations, sole proprietorships, and organizations serving as CHW employers under the RHTP CHW Employer Grant are not eligible.
