A 20% cap applies to IT and medical equipment costs charged to the award. Ineligible costs: leasehold improvements, salaries/fees for clinicians, recurring software/cloud costs, provider recruitment/quality incentives. Projects may be located in non-rural counties if they serve rural populations. Multi-site primary care operators must submit separate applications per site. Projects must conclude by June 30, 2031. Evaluation criteria: community need (Community Health Assessment preferred), project plan with logic model, evaluation approach, sustainability, and budget narrative.
Eligible applicants: governmental entities, nonprofits, for-profit organizations, community-based organizations, health systems, Federally Qualified Health Centers (FQHCs), and Tennessee state agencies serving rural populations in any TN county. Project location can be non-rural if it demonstrably serves rural healthcare access needs. Multi-site applicants may apply for multiple $500,000 primary care site awards (one application per site). Match: $40M requires 1:1, $10M–$20M requires 2:1, $5M and $2.5M require none; ARC at-risk/distressed counties exempt from match for ≤$20M. Leasehold improvements, clinical/provider compensation, and recurring software/cloud costs are ineligible.
