Summary

The Rural Healthcare Access Modernization Program (RAMP) is a Tennessee Department of Health initiative funded through TennCare Shared Savings, complementing the federal Rural Health Transformation Program (RHTP). The Tennessee General Assembly approved $104.4 million in TennCare Shared Savings to TDH in the FY27 budget (part of a $125M total appropriation; the remaining $20.6M funds a separate TennCare-administered Rural Health Clinic sustainability track). Grant period runs through June 30, 2031. RAMP funding priorities: Rural Capital Projects — awards of one $40,000,000, one $20,000,000, two $10,000,000, two $5,000,000, and four up to $2,500,000 (renovations, facility expansion, new construction; equipment/IT capped at 20% of grant amount); Competitive Grant Extensions — additional funding for eligible RHTP-aligned projects needing to continue or complete implementation; Primary Care Seed Funding — eight $500,000 awards and one $427,500 award (per-site cap $500,000) to establish or expand rural primary care, with focus on pediatric and obstetric care. Eligible applicants: governmental entities, non-profits, for-profits, and Tennessee state agencies serving rural populations in any of Tennessee's 95 counties. Grantees must attend a mandatory training within 90 days of contract execution covering invoicing/reimbursement procedures.

Eligibility

Eligible applicants include governmental entities, non-profits, for-profit organizations, and Tennessee state agencies serving rural populations in any of Tennessee's 95 counties. Multi-site operators with proven rural healthcare models may apply for multiple locations, subject to the per-applicant funding cap. Special consideration for distressed and at-risk counties applies: matching requirements are waived for these applicants.
