TennCare (State of Tennessee, Division of TennCare) Request for Application (RFA #31865-00020 Release #2, as amended) for the Community Health Worker (CHW) Infrastructure Project. Through the Tennessee Rural Health Transformation (RHT) initiative, TennCare will award up to 8 grants to eligible health care provider organizations that employ or plan to employ CHWs. Funding supports organizational infrastructure enhancement, adoption of the IMPaCT Community Health Worker model via structured technical assistance, and pursuit of Tennessee Community Health Worker Association (TNCHWA) Program Accreditation. Year 1 awards are tiered: $225,000 for organizations with 1-5 CHWs/supervisors, $450,000 for those with 6+; funds are allocated 67% for infrastructure and 33% for model/technical assistance. Grantees may earn a second funding year based on satisfactory performance, specifically completion of Section A.6 deliverables and maintaining/expanding CHW teams as outlined. Rural-serving organizations receive funding priority, but all TennCare provider applicants are eligible. Additional oversight, deliverables, and ongoing progress reporting are required for continued support.
Eligible applicants are Tennessee healthcare provider organizations serving TennCare members and employing or intending to employ Community Health Workers (CHWs), including primary care and outpatient providers. Priority is given to organizations serving rural TennCare populations, but non-rural-focused organizations are also eligible. Grantees must meet the minimum CHW/supervisor staffing for their tier by the end of the grant period but do not need to have all positions filled at application.
