Funds establishment or expansion of Umbrella Hub Organizations (UHOs) to increase participation in CDC-recognized chronic disease evidence-based programs—specifically the National Diabetes Prevention Program (NDPP) and Diabetes Self-Management Education and Support (DSMES)—among rural New Jersey populations. Grantees will build referral networks, implement EHR/practice system updates, train staff, conduct outreach to providers and potential participants, and coordinate with community-based organizations. Preference will be given to applicants serving areas designated as rural under federal, state, or Road Ruggedness Scale (RRS) definitions. Activities must target uninsured or underinsured individuals in eligible counties or census tracts. Funds may not support services reimbursable by Medicaid, Medicare, or private insurance.
Eligible applicants include rural hospitals and critical access hospitals; Federally Qualified Health Centers (FQHCs); rural health clinics; local health departments; nonprofit organizations serving rural communities; tribal health organizations; academic or healthcare-affiliated institutions partnering with rural entities; municipal and county governments; educational institutions; healthcare providers (such as clinics and health centers); community-based organizations (including libraries, community centers, and local service groups). Programs or activities must serve uninsured and underinsured individuals in federally/state/RRS-designated rural counties or census tracts in New Jersey. Applicants are not required to serve these populations exclusively but must demonstrate benefit to them. Priority is given to applicants serving federal HRSA-defined rural areas.
