RHTP RFA (DFHS26RWH) provides $2,000,000 in funding to support Community Health Worker (CHW) programs serving uninsured and underinsured populations in rural New Jersey. Eligible applicants include municipal/county governments, public and private non-profits, healthcare providers (clinics, health centers), community-based organizations, and tribal organizations, serving designated rural areas per federal, state, or Road Ruggedness Scale (RRS) definitions. Funded initiatives must prioritize recruitment, hiring, and training of CHWs, SDOH screening and referrals, partnership development, health education, care coordination, community outreach, and data reporting, with additional focus on underserved populations such as pregnant/postpartum individuals, CYSHCN families, and those with behavioral health needs. Grants range from $100,000 to $450,000 based on scope and staffing, with no first-year match required. Reporting requirements include monthly expenditure and quarterly progress reports, and all CHW staff must complete CLG-CHWI training.
Eligible applicants are municipal and county governments, public and private non-profit organizations, healthcare providers (e.g., clinics, health centers), community-based organizations, and tribal organizations serving uninsured and underinsured individuals in designated rural New Jersey counties and Rural Census Tracts (RCTs) defined federally, by state, or under the Road Ruggedness Scale (RRS). Applicants must operate or propose activities for target populations in Atlantic, Burlington, Cape May, Cumberland, Hunterdon, Mercer, Monmouth, Ocean, Salem, Sussex, Warren counties, and in RRS-designated RCTs in Bergen, Hudson, Morris, and Passaic.
