Supports community-based Medication Therapy Management (MTM) and tobacco cessation services integrated in rural New Jersey pharmacies, clinics, and health centers, with a focus on uninsured/underinsured populations and chronic disease management. Priority is given to interventions not covered by medical insurance, particularly for chronic conditions like diabetes, hypertension, heart disease, and hyperlipidemia. Funds staff, training, medical devices (screening and diagnostic tools, wearable biometric devices), outreach, virtual delivery, and technical assistance to pharmacies, FQHCs, and rural clinics. Programs must strengthen patient engagement, expand reach via telehealth, and foster partnerships for technical assistance and workflow standardization.
Eligible applicants include rural hospitals and critical access hospitals, Federally Qualified Health Centers (FQHCs), rural health clinics, local health departments, nonprofit organizations serving rural communities, tribal health organizations, academic or healthcare-affiliated institutions partnering with rural entities, municipal and county governments, educational institutions, healthcare providers such as clinics and health centers, community-based organizations (including libraries, community centers, local service groups), and tribal organizations. Applicants must operate or propose programs serving uninsured and underinsured individuals in federally/state/RRS-designated rural counties or census tracts as detailed in Appendices A, B, and C of the RFA. Applicants serving areas meeting federal HRSA rural definitions may receive priority consideration.
