Funds a multi-component, community-based youth violence prevention program in rural New Jersey: violence-prevention/conflict-resolution education, mentoring, family engagement workshops, and community outreach/coalition-building, delivered in schools, community centers, and mobile outreach settings. Required components include: (1) evidence-based violence-prevention and conflict-resolution curricula for youth, (2) mentoring for at-risk youth via trusted adults and near-peer models, (3) parent/caregiver workshops on trauma, substance misuse, and youth development, and (4) community outreach and collaboration, including youth-led service/leadership projects and partnerships with schools, faith organizations, law enforcement, and health providers. Programs must address access and engagement barriers, including transportation. Funding supports staffing, training, mentoring, evaluation, outreach, and indirect costs (capped at 10%). Programs must serve children and adolescents residing in designated rural counties or census tracts.
Eligible applicants include rural hospitals and critical access hospitals, Federally Qualified Health Centers (FQHCs), rural health clinics, local health departments, nonprofit organizations serving rural communities, tribal health organizations, academic or healthcare-affiliated institutions partnering with rural entities, municipal and county governments, educational institutions, community-based organizations (such as libraries and community centers), and tribal organizations. Applicants must operate programs or propose activities serving uninsured and underinsured individuals in the designated rural target-population counties and rural census tracts as defined by federal, state, or RRS metrics (Atlantic, Burlington, Cape May, Cumberland, Hunterdon, Mercer, Monmouth, Ocean, Salem, Sussex, Warren, plus select tracts in Bergen, Hudson, Morris, and Passaic).
