Expands access to chronic disease screening and education for rural New Jersey residents via walk-in and scheduled screenings, transportation, insurance enrollment, SDOH referrals, follow-up care coordination, and enrollment in evidence-based chronic disease self-management education programs.
Eligible applicants include rural hospitals and critical access hospitals; Federally Qualified Health Centers (FQHCs); rural health clinics; local health departments; nonprofit organizations serving rural communities; tribal health organizations; academic or healthcare-affiliated institutions partnering with rural entities; municipal and county governments; educational institutions; community-based organizations (e.g., libraries, community centers); and tribal organizations. Applicants must operate programs or propose activities serving uninsured and underinsured individuals in designated rural counties and census tracts as defined by federal, state, or Road Ruggedness Scale (RRS) criteria (counties: Atlantic, Burlington, Cape May, Cumberland, Hunterdon, Mercer, Monmouth, Ocean, Salem, Sussex, Warren and selected tracts in Bergen, Hudson, Morris, Passaic). Programs may interact with insured individuals but grant funds may not support services reimbursable by Medicaid, Medicare, or private insurance.
