Funds sustainable policy, systems, and environmental changes to reduce cancer burden and improve survivorship care in rural New Jersey. Supported activities include cancer prevention education and outreach, cancer survivorship workshops, clinical trial enrollment initiatives, faith-based women’s health partnerships focused on breast, cervical, and colorectal cancers, and policy advocacy. Funding is aligned with the NJ Comprehensive Cancer Control Plan and prioritizes underserved rural populations, especially uninsured and underinsured. Allowable costs include program staff, training and curriculum, evaluation, educational materials, in-state travel, and up to 10% indirect costs.
Eligible applicants include rural hospitals and critical access hospitals, Federally Qualified Health Centers (FQHCs) and rural health clinics, local health departments, nonprofit organizations serving rural communities, tribal health organizations, and academic or healthcare-affiliated institutions partnering with rural entities. Eligibility is limited to applicants serving uninsured and underinsured individuals in designated rural counties and census tracts as defined by federal, state, or Road Ruggedness Scale (RRS) criteria: Atlantic, Burlington, Cape May, Cumberland, Hunterdon, Mercer, Monmouth, Ocean, Salem, Sussex, Warren; plus ten (10) census tracts in Bergen, Hudson, Morris, and Passaic. Applicants serving areas meeting federal HRSA rural definitions may receive priority consideration.
