The RHT-AFA-06-12-2026-CCG County Community Grant, under West Virginia's Rural Health Transformation Program (RHTP), provides funding to support the implementation and scaling of EMS-led community paramedicine, mobile-integrated health, and treat-in-place models across all counties in WVOEMS Region 1 (McDowell, Mercer, Monroe, Raleigh, Summers, Wyoming). Eligible state-licensed EMS agencies or partnerships must deliver protocol-authorized on-scene assessment, treatment, care navigation, and referral services; coordinate with primary care, behavioral health, and social service providers; and participate in the Connected Care Grid data exchange and reporting system. Key program goals include reducing avoidable emergency department utilization, improving timely access to care, and demonstrating measurable improvements in cost, quality, and outcomes. This competitive sub-award is a one-year opportunity with a required detailed budget, milestone-based funding, and robust monthly/quarterly reporting obligations. A County Level Service Form specifying covered counties and estimated population served is required.
Eligibility
Eligible applicants are state-licensed EMS agencies able to provide community paramedicine services to all counties in WVOEMS Region 1. Applicants must operate in compliance with OEMS protocols, statewide quality, data, and reporting requirements. Applicants unable to cover all counties are encouraged to partner with other agencies to meet the regional coverage requirement. Registration with WV Secretary of State required; UEI required but full SAM.gov registration is not.

Applicants must:
Complete the County Level Service Form specifying proposed counties of service and estimated population covered.
Submit a Grant Funding Application via the WVOASIS VSS Portal, including all required attachments.
Attach a detailed line-item budget and a budget narrative.
Provide a narrative addressing organizational capacity, health outcomes impact, implementation plan, and sustainability.
Provide a grantee contact list (min. two contacts covering administrative, financial, project, and reporting roles).
Participate in required reporting (monthly/quarterly program and financial reports).
Register/verify with WV Secretary of State; obtain a UEI.
