the Rural Health Link (RHL): Public Transportation Collaboratives initiative. Up to $1.5 million will be awarded in Year 1 to eligible entities to establish and operationalize regionally-governed, multi-county public transportation collaboratives that coordinate non-emergency medical transportation (NEMT) across jurisdictions. Collaboratives must implement shared dispatch and scheduling infrastructure, formal governance structures, and partnership agreements, integrating operations with the State's data and mobility technology platform. Activities must focus on reducing fragmentation and duplication in current transportation systems while measurably improving access, reliability, and care outcomes for rural and underserved populations. Eligible applicants must demonstrate proven capacity for multi-county coordination, operational readiness for rapid launch, cross-sector collaboration, and a robust plan for data reporting and sustainability. Fund distribution is milestone-based, tied to State-approved deliverables and measurable outcomes in areas such as reduced avoidable emergency department utilization, improved treatment adherence, and increased workforce participation. All activities must align with the RHTP’s model for durable, statewide rural health infrastructure and are subject to CMS/HHS and State requirements. This funding may not be used as cost share or match in other federal or state programs without prior written State approval, nor may it duplicate existing value-based initiatives or programs.
Eligible applicants include regional transportation providers, public transit authorities, healthcare system-affiliated transportation entities, non-profit organizations, and other entities with demonstrated experience overseeing or delivering transportation services across multiple jurisdictions. Applicants must demonstrate operational capacity for multi-county transportation coordination, including dispatch operations, vendor management, and data reporting. Formal partnership, governance structures, and a readiness plan for integration with the State’s data and mobility technology platform are required. Applicants must be prepared to execute or actively develop cross-county governance agreements and show evidence of readiness to manage milestones, reporting, and compliance per State and CMS requirements.
