the state. The goal is to remove transportation and care navigation barriers for rural West Virginians by designing, launching, and running integrated coordination infrastructure, emphasizing data-driven operational improvements, comprehensive partnerships, and long-term sustainability. Up to $7.1 million (shared pool) will be awarded as first-year subawards to eligible public or private organizations with demonstrated program coordination experience. Funded activities will include creation or expansion of regional hubs, staff/dispatcher training, standardized workflows, development of contractual and operating partnerships (with transportation providers, care navigators, health and community organizations), deployment of mobility coordinators, and full integration with existing and future State data/reporting platforms. Funding and disbursement are milestone-based, with no cost sharing required. Programs must clearly measure and report improvements in healthcare access, appointment completion, reduced emergency department use, and system efficiency, and avoid duplication of existing programs. Awards cover activities and invoicing through September 30, 2027. Applications are evaluated based on completeness, alignment with RHTP and CMS objectives, measurement/reporting, budget justification, organizational capacity, community partnerships, regional impact, data integration, compliance, and post-award sustainability.
Eligible applicants include public or private entities with demonstrated experience operating regional or statewide coordination programs—including, but not limited to, healthcare access, transportation coordination, dispatch operations, or care navigation for rural or underserved populations. This may include nonprofit organizations, governmental or quasi-governmental entities, health systems, or other organizations capable of supporting operations and integrations. The State may award funding to one or more grantees based on application quality and geographic coverage.
