Funded activities must align with CMS-approved Rural Health Transformation Program (RHTP) objectives, reducing chronic disease burden, increasing workforce participation, and improving measurable healthcare outcomes in rural communities. The initiative supports operational scalability, real-time data integration, and post-award sustainability, awarding funds based on performance measures and milestone-based deliverables. All data generated through this program must be accessible to the State. Applicants must present credible measurement and reporting plans and ensure no duplication of scope with existing mandated programs. The grant period ends September 30, 2027, with funding based on State-approved milestones, and does not require cost sharing. Applications are evaluated on completeness, alignment with RHTP goals, budget justification, organizational capacity, rural community partnerships, service area impact, and compliance.
Eligible applicants include public or private entities with demonstrated experience operating regional or statewide coordination programs, including healthcare access, transportation coordination, dispatch operations, or care navigation for rural or underserved populations. This includes, but is not limited to, nonprofit organizations, governmental or quasi-governmental entities, health systems, or other organizations capable of supporting operations and integrations. The State intends to award funding to one or more grantees based on application quality and geographic coverage.
