the Rural Health Transformation Program, this opportunity emphasizes expanding access to advanced cardiovascular diagnostics, interventions, disease management, rehabilitation, tele-cardiology, and provider collaborations. Applicants may apply as: (1) regional Hub-and-Spoke networks for advanced cardiovascular services, (2) individual organizations expanding Hub capacity in underserved rural regions, or (3) networks focusing on support and sustainability for rural Spokes emphasizing prevention and chronic disease management. Applications must demonstrate strong formal partnerships with at least two Spoke facilities (minimum network size: three organizations), all meeting the geographic and operational requirements as set forth in the RFP. Key activities under this RFP include establishing collaborative referral agreements, recruiting providers, acquiring equipment, undertaking minor renovations, and strengthening maternal health care delivery. Networks must increase access to services, address persistent rural care gaps, promote innovative telehealth models, and reduce health disparities. This initiative is part of the larger Iowa Healthy Hometowns Project and leverages $33,361,333 in first-year funding.
Eligible applicants include public, private, nonprofit, for-profit, academic institutions, or governmental entities physically located in Rural Iowa (as defined by the Rural Health Grants Eligibility Analyzer). Applicants must have been open and providing services in Iowa as of January 1, 2026 and currently offer services at time of application. Applicants must submit a mandatory letter of intent by October 19, 2026, specifying their program option and naming anticipated network members. Each application must represent a Hub and Spoke Network with at least three Iowa-based members (including the applicant). Additional eligibility criteria apply per program option. Recipients of technical assistance contracts under COMPADM26002 or COMPADM26003 may apply if staff non-disclosure agreements are provided by the LOI deadline.
