Requirements Checklist

• Applicants must:
• Submit a mandatory Letter of Intent by August 10, 2026, 4:00 PM CT via email (cc all required addresses).
• Submit Non-Disclosure Agreements (if applicable, for TA awardees) by August 10, 2026, 4:00 PM CT via email.
• Identify a physical service location in a Rural area (per eligibility analyzer definition).
• Propose a new Co-Location Site or a meaningful expansion of an existing one by adding at least two new, unaffiliated service delivery entities.
• Identify at least three separate and distinct service delivery entities (at least one must provide chronic disease services; priority for all three entities being unaffiliated; priority for including a health-related social needs provider; and priority for including only one medical care provider among the three entities).
• Include signed MOUs from each partner documenting: commitment to collaborate; reference to working toward legal agreement within six months; mutual budget agreement; signatures from authorized officials.
• Submit all required forms and documentation solely via the IowaGrants portal by the application deadline (one application per site).
• Sign and execute participation agreements for Iowa’s Health Information Exchange.

Required attachments

• Signed memorandums of understanding (MOUs) with each service delivery entity partner (must describe commitment to collaborate, outline mutual budget agreement, be signed by authorized officials, and reference working toward legal agreement within six months)
• Budget agreement included in MOUs
• Disclosure and other forms as required in RFP
• Non-Disclosure Agreements (if applicable; required for TA awardees per RFP Section 1.03)

Submission Instructions

Submit all application forms and required attachments via the IowaGrants Electronic Grant Management System (https://iowagrants.gov/) by 12:00 PM CT, September 1, 2026. Letters of Intent and Non-Disclosure Agreements (if applicable) must be emailed to ryan.roovaart@hhs.iowa.gov (cc shannon.garland@hhs.iowa.gov) by August 10, 2026, 4:00 PM CT. All written questions must be submitted via IowaGrants.
