Healthy Hometowns: Mobile Integrated Health (RFP # PHTHOET27001)

Summary

The program supports EMS agencies or community-based clinical providers in launching or broadening treat-in-place care, telehealth-enabled assessments, chronic disease management, post-discharge follow-up, prenatal and postpartum home-based support, medication reconciliation, and other in-home interventions. Applicants may propose to initiate a new MIH program or expand an existing MIH program to additional rural counties. The goal is to improve health outcomes for rural Iowans, reduce avoidable emergency department and inpatient utilization, and strengthen care capacity through coordinated, community-based services. This federally funded initiative, part of the CMS Rural Health Transformation Program (CMS-RHT-26-001), prioritizes projects that cover multiple rural counties and leverage telehealth and care coordination tools.

Eligibility

Eligible applicants include public, private, nonprofit, for-profit, academic institutions, or governmental entities currently providing Iowa Authorized EMS Services or community-based clinical services. Applicants must serve at least one county with rural eligibility determined by the Rural Health Grants Eligibility Analyzer. All MIH services must be non-reimbursable under existing insurance. Applicants must use the ImageTrend Data System Community Care module for reporting and participate in the state's BEMTS pilot. Funds may not be used to purchase, lease, rent, or maintain vehicles including ambulances, aircraft, vans, cars, or any motorized vehicle for medical or transportation use.
