Indiana RHTP07 Teleconsult and Telehealth Landscape Assessment

Eligibility

Respondents must have a valid State of Indiana Bidder Profile, be registered with the Indiana Secretary of State, and demonstrate experience in statewide assessment, rural health, and/or program evaluation. Compliance with Indiana’s Minority and Women’s Business Enterprise (MBE/WBE) and Indiana Veteran Owned Small Business (IVOSB) participation goals is required.

Requirements Checklist

• Respondents must: 1) Complete and submit all required attachments including Indiana Economic Impact Form (Attachment C), Cost Proposal (Attachment D), MBE/WBE Commitment (Attachment A), IVOSB Commitment (Attachment A1) with appropriate sub-contractor letters; 2) Submit both redacted (public) and unredacted proposals if confidential information is included per the Access to Public Records Act; 3) Engage a defined percentage of identified stakeholders as mutually agreed with the State; 4) Submit monthly and quarterly status/success reports; 5) Meet participation goals (8% MBE, 11% WBE, 3% IVOSB of total bid amount).
