Requirements Checklist

• For each Program Option applied to, applicants must submit: (1) a standalone Narrative describing intervention design, engagement and adherence strategies, scalability and sustainability, and data integration; (2) a Detailed Line Item Budget (DLIB) using the State template; (3) a Budget Narrative-Justification; (4) a Grantee Contact Information Form; and (5) a County-Level Service Form. Administrative costs are capped at 10% of RHTP funds. In-kind match (e.g., donated staff time, facility use) is not allowed; all match must be cash, committed, and verified prior to disbursement. Each application must identify a single prime applicant (accountable lead), specifying clear roles and accountability for any coalition or collaborative. All proposed interventions must track and deliver performance/outcomes as described in the AFA. Payments are tied to required milestones and outcome indicators.
