The State of West Virginia, through its Department of Health's Rural Health Transformation Program (RHTP), announces this competitive Announcement of Funding Availability (AFA) for qualified organizations to design, implement, and operate clinically integrated nutrition and lifestyle intervention programs statewide, targeting rural, high-risk populations. Supported by CMS/HHS under the Personal Health Accelerator (PHA) flagship initiative, funded programs must deliver measurable health improvements in key domains including chronic disease reduction, enhanced workforce participation, and reduced healthcare utilization and costs. All programs must establish provider referral pathways; deliver structured, evidence-based interventions; track participant engagement and outcomes; regularly report performance metrics; and integrate data with the state’s Connected Care Grid Data Spine. Funding is milestone-based, time-bound, and subject to performance accountability, with clear requirements for scalability and long-term sustainability through payer/value-based care alignment. Programs will be held to administrative cost limitations, matching/cost share preferences, and stringent privacy/security standards. Applicants may propose one or more of seven defined program options spanning metabolic & cardiovascular health, respiratory health, SUD, and senior care, each with specific engagement, outcome, and reporting benchmarks. The initiative aims to engage approximately 23,000–25,000 rural West Virginia adults in Year 1 and up to 120,000 cumulatively over Years 1–5. Funded solutions must meet defined performance targets within State-established timelines, including participant-level improvements in clinical indicators (e.g., A1C, BMI, hypertension rates). Awards are subject to milestone reduction or reallocation for non-compliance. Programs must demonstrate ability to scale statewide and a credible plan for post-award financial sustainability. All awarded funds must be expended and invoiced by July 31, 2027.
Eligible applicants: Healthcare providers, community-based organizations, non-profits, payers (including Medicaid-aligned entities), integrated delivery systems, FQHC/CHCs, hospitals/health systems, local health alliances, home health agencies, pharmacies, multi-partner collaboratives with a single accountable non-governmental lead, and other qualified vendors with demonstrated capacity to scale nutrition and lifestyle intervention programs. State agencies are NOT eligible. Preference for applicants with demonstrable ability to impact population health statewide, formal collaborations with WV-based coalitions, and those able to provide a committed, verifiable private funding match (1:1 preferred; no in-kind match allowed).
