This Announcement of Funding Availability (AFA) seeks applications for the Rural Health Transformation Program (RHTP) – Health to Prosperity: Restoring WV’s Workforce through Health-to-Work Programs for Substance Use Disorder (SUD). The program aims to reduce unemployment and avoidable absenteeism in West Virginia by supporting the implementation and expansion of return-to-work (RTW) and recovery-to-work coordination models for individuals whose substance use disorders are barriers to employment. Funded activities must focus on strategies such as liaison-based coordination, employer engagement, clinical and recovery support referrals, standardized RTW planning, and robust data reporting to the statewide RHTP data spine. The grant emphasizes preventative, root-cause approaches and requires demonstration of scalability and sustainability beyond Year 1. Funding is performance-based with reimbursement tied to milestone deliverables, and grantees are expected to coordinate across RHTP’s other initiatives to maximize outcomes and data integration. Applicants must demonstrate both operational readiness and governance capacity for rapid program launch and ongoing compliance with reporting requirements. This opportunity is funded via CMS/HHS and supports Governor Morrisey’s ‘Four Pillars of a Healthy West Virginia’ and the ‘Making America Healthy Again (MAHA)’ initiative.
Eligible applicants include commercial organizations or other organizations with the capacity to support return-to-work programs; health systems or provider-affiliated entities; state universities or colleges; workforce or employment-support organizations; and regional or community-based organizations with demonstrated employer engagement capacity. Applicants must demonstrate operational capacity and a governance structure to deploy field-based staff, coordinate across healthcare and employer settings, and meet State reporting and compliance requirements. Employers can participate as implementation partners but are not eligible applicants for direct funding.
