Tier 3 is designed for hospitals with established Hospital at Home programs that have already achieved CMS Acute Hospital Care at Home (AHCAH) approval and are focused on expansion, optimization, and long-term sustainability. Funding supports enhancing program performance and extending access to inpatient-level care, particularly in rural communities. Maximum award is $2,000,000. HARI's role is limited to program oversight, statewide coordination, reporting guidance, and peer learning support -- not hospital-specific operational management.
Eligible applicants are Rhode Island acute-care hospitals with an existing CMS Acute Hospital Care at Home (AHCAH) waiver approval and an operational Hospital at Home program. Hospitals must demonstrate program maturity and readiness to scale, with a plan for extending inpatient-level care access, especially in rural communities.
