Hospital at Home Program Implementation and Scale Grant Opportunity — Tier 2 (Program Development & Launch)

Summary

Tier 2 supports hospitals that have completed foundational planning and are preparing for or pursuing CMS AHCAH approval. Funding may be used for program development, infrastructure and workforce development, CMS waiver preparation, and initial program launch activities. Maximum award is $2,000,000, reimbursed upon milestone completion. Technical assistance/consultant support (available to Tier 1) is not provided for Tier 2 -- hospitals are responsible for launch activities.

Eligibility

Eligible applicants are Rhode Island acute-care hospitals that are licensed and in good standing. Hospitals must be positioned to pursue or have initiated efforts toward CMS Acute Hospital Care at Home (AHCAH) waiver approval, and be prepared to implement new Hospital at Home programs or launch new phases of Hospital at Home care delivery as outlined for Tier 2.

Requirements Checklist

• Applicants must: (1) Complete the Tier 2 application in full, including project narrative, budget, and work plan; (2) Detail their intended plan for AHCAH waiver application/approval; (3) Demonstrate readiness through attestation and documentation detailed in the NOFO; (4) Submit documentation of board approval, letters of support, and commitment to reporting; (5) Provide implementation and workforce development plans; (6) Agree to all milestone-based deliverables and post-award reporting requirements described in the NOFO.
