Hospital at Home Program Implementation and Scale Grant Opportunity — Tier 1 (Organizational Readiness & Planning)

Summary

Tier 1 of the Hospital at Home Program Implementation and Scale Grant Opportunity provides non-renewable, milestone-based funding of up to $1,000,000 for Rhode Island acute-care hospitals to undertake organizational readiness and implementation planning for Hospital at Home programs. Activities supported include feasibility assessments, operational readiness, detailed workplans, and strategic planning aimed at meeting CMS Acute Hospital Care at Home (AHCAH) requirements. Awardees receive technical assistance and consulting support, participate in program assessments, and must complete a Readiness Report and Strategic Recommendation. Only hospitals not yet CMS AHCAH-approved or lacking a prior readiness assessment are eligible for Tier 1 funds. Deliverables, milestones, and reporting requirements are detailed in the NOFO. Funding will be reimbursed post-milestone completion, with program participation and milestone achievement affecting eligibility for future funding tiers. Eligibility

Eligible applicants are Rhode Island acute-care hospitals that have not yet completed a Hospital at Home readiness assessment and do not have CMS Acute Hospital Care at Home (AHCAH) approval at the time of application.

Requirements Checklist

• Applicants must submit a planning narrative and project workplan per NOFO guidance, demonstrate executive support via a signed attestation, participate in technical assistance and assessments, provide necessary organizational and IT information, review readiness findings with a consultant, develop a Readiness Report and Strategic Recommendation, and complete post-award reporting.
