The Healthy Horizons RFA under New Mexico's Rural Health Transformation Program (RHTP) funds Regional Hub Organizations to expand specialty, maternal, behavioral health, and chronic disease services in rural, frontier, and tribal communities. Projects must demonstrate sustainability, include cross-sector partnerships, and comply with clarified budgeting and allocation methodologies (see amendment and FAQ documents). Maternal health allocations apply specifically to Region 3. Applications must document organizational structure and partner roles; future subrecipients need not be fully identified at submission.
Eligible applicants are New Mexico-based public, nonprofit, or tribal organizations that operate licensed healthcare facilities, clinics, or provider groups in rural, frontier, or tribal areas. For-profit entities and organizations outside New Mexico are not eligible. Food banks and community-based organizations may participate as partners or subrecipients if eligibility criteria are met (see FAQ).

Submit complete application electronically via the NM HCA RHTP applicant portal (https://hca.nm.gov/rhtp/apply). Applications must be received by July 6, 2026, 5:00 PM MST. Late or incomplete applications will not be accepted.
