The Tennessee Department of Health's Rural Health Transformation Program (RHTP) offers competitive grants through the Healthcare Resiliency Program: Make Rural Tennessee Healthy Again Procurement Opportunity (RFA #34320-20127). This initiative aims to advance healthcare access, quality, and outcomes in rural Tennessee, with an emphasis on hospital sustainability, modernization of health technology, and enhanced care for mothers, children, and older adults. Funded by CMS/HHS, this RFA supports locally-driven, innovative solutions to reverse rural hospital decline and foster sustainable health system transformation. Supplemental webinars, information sessions, application materials, and updated budget templates are available via the TDH Procurement Opportunities website and the Partner Portal. All interested providers are encouraged to download the most current RFA documents and contract templates when preparing their submissions.
Eligible applicants include rural hospitals, Federally Qualified Health Centers, non-profit and community-based organizations, and other healthcare providers registered with the State of Tennessee.
