the Rural Health Transformation Program. This competitive Request for Applications solicits New Hampshire-based nonprofit, for-profit, and community-based organizations—including licensed EMS units and partnerships with such units—to stabilize, strengthen, and expand rural EMS systems and Mobile Integrated Health (MIH) services. Organizations may request funding under three components: Component 1: Stabilize & Strengthen Rural EMS Systems (up to $25,000); Component 2: MIH Expansion of Existing Programs ($100,000–$400,000); Component 3: MIH New Regional Programs ($300,000–$2,000,000). Activities may include technology upgrades, workforce development, care model innovation, and regional collaborations. The funding period runs through September 30, 2027, with the possibility of continued support through September 30, 2031 contingent on satisfactory performance, available funding, and continued GO-NORTH authorization. Grant writing support, technical assistance, and information sessions are available. All relevant application, eligibility, and reporting requirements are detailed in the RFA.
Eligible applicants include all New Hampshire-based nonprofit, for-profit, and community-based organizations in good standing with the NH Secretary of State. Applicants must be: licensed NH EMS units; organizations without EMS licensure but with a licensed NH EMS unit formally involved in the application; or organizations actively pursuing NH EMS licensure (with required documentation). Ineligible entities include individuals or organizations with any ongoing legal proceedings involving NH or other government bodies. Duplicate activities already covered by other GO-NORTH or external funding sources, and certain activities (such as capital facility expenditures, research, lobbying, or direct employee cash benefits for recruitment/retention) are not eligible.
