Expanding Worksite Clinics for On-the-Job Support

Summary

the Rural Health Transformation Program (RHTP). Grantees will design, implement, and scale employer-aligned clinics delivering preventive, occupational health, chronic disease management, care coordination, and closed-loop referrals to improve workforce health, retention, and return-to-work outcomes for adults with high-prevalence, employment-disrupting chronic conditions. Funded projects must emphasize early identification of health risks, scalable infrastructure, non-duplicative services, milestone-linked accountability, and integration of program data with the State data spine. Funding cannot reimburse routine clinical services otherwise payable by insurance or employer health plans and may not be duplicative or supplanting existing support. All clinics must be operational within 90 days of award and achieve meaningful service volume and workforce impact by July 31, 2027, with clear processes for employer participation, compliance with CMS and State requirements, and eventual financial sustainability.

Eligibility

Eligible applicants include: (1) State institutions of higher education; (2) health systems and provider organizations (e.g., integrated systems, occupational health organizations); (3) for-profit or nonprofit local or regional organizations with statewide scale capacity; (4) consortia or partnerships (single prime applicant). Demonstrated experience in multi-employer worksite clinic or occupational health models is required. Employers may participate as implementation partners but are not eligible for direct grant funding. Applicants must provide at least two grantee contacts and have active SAM.gov/UEI registration.
