the state’s Rural Health Transformation Program (RHTP), funded via CMS Assistance Listing 93.798. The selected vendor will assess implementation, impact, and sustainability of all funded RHTP initiatives. Key tasks include: designing and executing an evaluation plan, collecting and analyzing quantitative and qualitative data, engaging with contracted Hubs and subrecipients, supporting federal/state reporting requirements, and communicating results to GO-NORTH, CMS, and stakeholders. Deliverables include evaluation plans, quarterly and annual reports, mid-course recommendations, performance measurement tools, and ongoing technical engagement with partners. The contract is funded up to $25,000,000. The period of performance commences upon Governor & Council approval through September 30, 2031, with a potential one-year extension at state discretion. All activities must comply with 2 CFR Part 200 and applicable state and federal requirements.
Eligible applicants are professional organizations with demonstrated experience evaluating complex, multi-partner, federally funded initiatives. At least three years’ experience in healthcare or public health evaluation is required. Applicants must evidence capacity for in-state New Hampshire presence. Per RFP, organizations designated under RSA 21-I:11-c are ineligible. Individuals and non-entity applicants are not eligible. Preference will be given to vendors demonstrating significant experience in healthcare and public health evaluation.
