The Tennessee Memory Care Assessment Network (MCAN) is a statewide hub-and-spoke initiative under the Tennessee Rural Health Transformation Program (RHTP). MCAN is designed to expand access to memory assessment and diagnostic services, advance early dementia detection, and promote provider education throughout rural and underserved Tennessee communities. Through a single-award, one organization will manage MCAN implementation, subcontract with four to five rural clinics (five satellite sites minimum), develop and disseminate standardized dementia diagnosis protocols, and coordinate care navigation and referral to Dementia Navigators. Additional responsibilities include IT platform development for secure health information sharing, evaluation data infrastructure, provider engagement, and participant/caregiver referral management. The maximum total funding is $16.5 million for MCAN over a 50-month project period, with annual and clinic-expansion allocations as stipulated. Quarterly reporting and compliance with TDH and federal requirements are mandatory. Applicants must demonstrate organizational capacity to deliver required network activities; detailed receipts, payment overviews, and compliance documentation are required with invoices.
Eligible applicants include health systems, universities, specialty providers, health clinics, rural hospitals, Federally Qualified Health Centers (FQHCs), non-profit organizations, community-based organizations, and other eligible healthcare partners. All applicants must be registered with the State of Tennessee.
