Summary

The West Virginia Department of Health Rural Health Transformation Program (RHTP) Connected Care Grid pillar is soliciting competitive applications from state-licensed EMS agencies to implement or expand community paramedicine, mobile integrated health, and treat-in-place models within WVOEMS Region 1. The aim is to reduce avoidable emergency department utilization, improve access to care in rural areas, and integrate EMS with regional care networks. Funded activities include protocol-authorized on-scene assessment and treatment, care coordination, participation in statewide data and reporting infrastructure, workforce development, and procurement of approved equipment and technology. This is a milestone-based, one-year sub-award totaling $1,033,840, with payments tied to achievement of key deliverables across all counties in Region 1 (McDowell, Mercer, Monroe, Raleigh, Summers, Wyoming). Applicants must ensure compliance with all reporting, data, and partnership requirements and demonstrate ability to serve the full regional footprint, either independently or in partnership.
