Alabama RHTP Cancer Digital Regionalization Initiative

Summary

The program advances objectives of the CMS Rural Health Transformation (RHT) Program and emphasizes four strategy areas: (1) expanding proven cancer detection capabilities in rural healthcare settings; (2) establishing and operating regional hubs for cancer screening to coordinate services and referrals; (3) deploying mobile cancer screening units in underserved rural areas; and (4) community activation, outreach, and education to boost awareness and participation in screening. Eligible projects must clearly show benefits to rural Alabama through improved access, stronger regional service coordination, and the creation of community partnerships aimed at better health outcomes. Funding is limited to direct project costs, including technology, equipment, mobile unit operations, outreach, and education; administrative or indirect costs are not allowed. Projects are competitively selected as subawards and are subject to federal, state, and program requirements under ADECA’s CMS cooperative agreement, with quarterly and final reporting, procurement controls, and compliance monitoring required.

Eligibility

Eligible applicants include hospitals, health systems, rural hospitals, rural health clinics, federally qualified health centers, and other healthcare or community organizations capable of implementing activities that support access to cancer screening services and related education/engagement. Applicants must have the organizational capacity to align with Cancer Digital Regionalization Initiative goals and comply with award requirements. A valid Unique Entity Identifier (UEI) and active SAM.gov registration must be maintained throughout the award. Applicants may submit multiple applications for separate and defined projects, and do not need a rural location but projects must directly benefit rural Alabama patients, providers, or systems.

Requirements Checklist

• Applicants must submit: a current Certificate of Existence from the Alabama Secretary of State (issued within 30 days), current SAM.gov registration, IRS Form W-9, Alabama Vendor Disclosure Statement, written policies (conflicts of interest, financial management, internal controls, procurement, standards of conduct), financial statements for 3 completed fiscal years (audited if available), Single Audit or program-specific audit reports for 3 completed fiscal years, any audit findings/management letters/corrective action plans, completed ADECA project plan (template), grant budget workbook and narrative (with supplemental schedules if required), documentation of matching/in-kind contributions (if claimed), and relevant partner agreements/commitments. Costs claimed must be direct, allowable, related to the approved project, and consistent with 2 CFR 200 and CMS RHT requirements. Indirect and administrative costs are not allowable.
