Alabama RHTP Maternal and Fetal Health Initiative

Summary

Supports practical, scalable strategies that strengthen maternal healthcare delivery systems across rural Alabama through the establishment and enhancement of regional maternal and fetal referral hubs, expansion of telerobotic ultrasound capacity, and facility readiness upgrades to improve access and outcomes for expectant mothers and infants.

Eligibility

Eligible applicants include hospitals, critical access hospitals, rural emergency hospitals, rural health clinics, federally qualified health centers, health systems, and healthcare organizations capable of carrying out a Maternal and Fetal Health Initiative project and complying with all applicable award requirements. Applicants do not need to be physically located in a rural area but must demonstrate that the project is designed to benefit rural Alabama communities, expectant mothers, infants, healthcare providers, or facilities in regional referral networks.

Requirements Checklist

• Applicants must have a valid UEI and maintain an active SAM.gov registration throughout the award. Applications must include all required documentation such as a current Certificate of Existence from the Alabama Secretary of State (within 30 days of application), SAM.gov registration, IRS Form W-9, Alabama Vendor Disclosure Statement, written policies and procedures (conflict of interest, financial management, procurement, standards of conduct), financial statements for the past three fiscal years, single/program-specific audit reports, project plan (ADECA template), grant budget workbook, documentation of matching funds (if applicable), and any other project-specific or required supporting materials. Indirect/administrative costs may not be included.
