Applicants may propose diverse models, with some past success using embedded Community Health Workers (CHW) in school districts, but proposals are not limited to this model.
Qualified entities capable of providing youth tele-behavioral health services in Maine, with competence to serve youth ages 12-18 in rural and school-based settings.
