Nursing Home Telehealth: Nursing Facilities

Summary

This grant track ("Nursing Facilities") funds participating nursing facilities to adopt standardized telehealth workflows for timely specialty consultation, aiming to support up to 15 participating SNFs and up to 100 geriatric specialty/psychiatry consultations per month, reduce avoidable emergency department visits and hospital transfers, and support aging in place. A separate, not-yet-open track on the same page ("Specialty Telehealth Service Provider", a Partner Service Agreement for telehealth vendors) has dates listed as TBD and is not covered by this record.

Eligibility

Eligible applicants must be nursing facilities operating in Maine, demonstrating organizational readiness for telehealth implementation (technology readiness, workflow integration, staff training, resident participation, scheduling coordination, reporting, monitoring, evaluation, and quality improvement). Applicants must support resident privacy, consent, and HIPAA-compliant workflows, and maintain applicable state/federal licensing. Preference may be given to facilities serving rural, underserved, uninsured, and/or MaineCare populations.
